Survey Vendor Application Process

Survey vendors interested in becoming a CMS-approved In-Center Hemodialysis CAHPS Survey vendor will need to meet a number of business requirements, which are described in the ICH CAHPS Survey Administration and Specifications Manual that is available here.

The application and approval processes will consist of the following:

  • Completing and submitting a Vendor Registration and Application and demonstrating experience conducting mail, telephone, or mixed-mode surveys. If you are interested in becoming approved to administer the ICH CAHPS Survey, please contact the ICH CAHPS Coordination Team via email for more information.
  • Completing the self-paced Introduction to the In-Center Hemodialysis CAHPS Survey Webinar Training and successfully completing the training certification, and participating in all ICH CAHPS Survey Vendor Update Webinar Training sessions. Both trainings are held in February of each calendar year. An announcement will be posted each December with the specific training dates.
  • Completing and submitting a Quality Assurance Plan, which survey vendors will be required to complete and submit after their first ICH CAHPS data submission and on an annual basis thereafter.
