Revised OMB Expiration Date on Mail Questionnaire for the 2022 Fall Survey Period

The ICH CAHPS Coordination Team would like to alert ICH CAHPS Survey vendors of the following change that will be made to the mail version of the ICH CAHPS Survey, in all approved languages:

• The OMB expiration date (which is required to be printed under the stand-alone OMB number in the upper right-hand corner on the cover or first page of the mail survey) should be revised to display as: “Expiration Date: September 30, 2025”.
• For reference, the current mail survey displays this OMB expiration date as: “Expiration Date: December 31, 2022”.

All ICH CAHPS Survey vendors must make this change in their survey materials before the 2022 ICH CAHPS Fall Survey’s 2^nd mailing date of December 2, 2022 (the 2^nd mailing for mail-only client facilities). However, if a vendor has not yet printed the mail surveys for the upcoming November 4, 2022 1^st mailing date, please revise the OMB expiration date at this time.

The revised final ICH CAHPS Survey mail survey in all approved languages, with this new expiration date, will be made available on the ICH CAHPS website in early February 2023.
