The Coordination Team has finalized the schedule for the 2015 In-Center Hemodialysis CAHPS (ICH CAHPS) Fall Survey. The data collection schedule and the dates for some key activities related to the 2015 Fall Survey are shown below.

1 Only ICH facilities that will be switching to a different survey vendor and those that will be participating in the ICH CAHPS Survey for the first time will need to complete the online vendor authorization form.
2 If a facility has already submitted a Facility Non-Participation Form in calendar year (CY) 2015, it is still valid for the 2015 Fall Survey. Only facilities that decide NOT to administer the ICH CAHPS Survey in 2015 that have not already completed a 2015 Facility Non-Participation Form need to complete this form.
Please note that although the schedule indicates that vendors should clean and process data from January 12 – January 26, 2016, vendors should be cleaning and processing data as it is received throughout the data collection period.
