REVISED Data Collection Schedule for the 2015 ICH CAHPS Spring Survey

The Coordination Team has revised the schedule for the 2015 In-Center Hemodialysis CAHPS (ICH CAHPS) Spring Survey to allow more time for vendors to receive undeliverable prenotification letters prior to the first mailing. The revised data collection schedule for the 2015 Spring Survey is shown below.

1Only ICH facilities that will be switching to a different survey vendor and those that will be participating in the ICH CAHPS Survey for the first time will need to complete the online vendor authorization form.
