Submission of Facility-Specific Questions

As noted in an e-mail message sent to ICH CAHPS Survey vendors on June 3, 2014, the deadline for submitting facility-specific supplemental questions to the ICH CAHPS Coordination Team for review and approval is 5:00 PM Eastern Time on June 30, 2014.

Please note that the Coordination Team will accept facility-specific supplemental questions for review after the June 30th deadline; however, approval of those questions may not be granted in time for the questions to be included in the CY2014 ICH CAHPS Fall Survey. Survey vendors should work with their client ICH facilities to complete the vendor authorization and submission of facility-specific questions as soon as possible to avoid a delay in approval of those questions.
