Vendor Registration and Vendor Application Now Available

The Centers for Medicare & Medicaid Services (CMS) will begin the national implementation of the In-Center Hemodialysis CAHPS (ICH CAHPS) Survey in calendar year (CY) 2014. In-center hemodialysis facilities must contract with a survey vendor that has been approved and trained by CMS to administer the survey on their behalf in CY2014 as part of the requirements for the ESRD quality incentive program (QIP) for payment year (PY) 2016 and beyond.

Survey vendors seeking to become a CMS-approved ICH CAHPS Survey vendor must complete the following initial steps before vendor training occurs:

  1. Designate a staff member to be the vendor’s ICH CAHPS Survey Administrator. The Survey Administrator will complete the vendor registration and application process.
  2. The vendor’s designated ICH CAHPS Survey Administrator must complete the online Vendor Registration (enter the organization’s name and the designated ICH CAHPS Survey administrator’s name and email address). As part of the registration process, create a username and password to be used for accessing private links and forms on the ICH CAHPS Website.
  3. Complete and submit the Vendor Application which will be available via a Vendor Dashboard page. You will be directed to your Vendor Dashboard once you submit the Vendor Registration.
  4. Print, complete, notarize, and mail the ICH CAHPS Survey Administrator Consent Form to the ICH CAHPS Coordination Team at the address listed on the top of the form. The Consent Form will be available via your Vendor Dashboard once you have submitted the Vendor Application.
