Survey Vendor Application Process

Survey vendors interested in becoming a CMS-approved In-Center Hemodialysis CAHPS Survey vendor will need to meet a number of business requirements, which are described in the ICH CAHPS Survey Administration and Specifications Manual that is available here.

The application and approval processes will consist of the following:

  • Completing and submitting a Vendor Registration and Application and demonstrating experience conducting mail, telephone, or mixed-mode surveys. The Vendor Registration form is available under the “For Vendors” tab on this website. If you are interested in becoming approved for the CY2016 ICH CAHPS Fall Survey, please contact the ICH CAHPS Coordination Team via email before January 15, 2016.
  • Participating in the Introduction to the In-Center Hemodialysis CAHPS Survey webinar training session and successfully completing the training certification, and participating in all Update training sessions. The next Introduction to the ICH CAHPS Survey webinar training session is scheduled for February 2016.
  • Completing and submitting a Quality Assurance Plan, which survey vendors will be required to complete and submit after their first ICH CAHPS data submission and on an annual basis thereafter.
