As Hurricane Florence quickly approaches the East Coast, RTI International, which is headquartered in North Carolina, could experience intermittent power outages once the hurricane makes landfall on Thursday, September 13, 2018 and in the days after.

This announcement includes a list of the data quality issues that the Coordination Team (CT) detected in the data from the 2018 Spring Survey and issues we observed during site visits to survey vendors during the 2018 Spring Survey. As you prepare for the 2018 Fall Survey data collection period, please apply appropriate quality control measures so that you can detect and avoid these errors and data problems in all upcoming survey periods.

Data Quality Reminders

2018 Spring data submitted by vendors looked great! The one general issue found involved the assignment of final disposition codes.

To better assist ICH CAHPS Survey vendors, the Coordination Team has implemented an automated email to vendors when a facility adds an end date for that vendor on their Vendor Authorization Form. The automated email will list the CCN(s) that updated their Vendor Authorization Form with an end date. If vendors are not aware of this end date, which indicates that the facility will no longer be contracting with the vendor for the ICH CAHPS Survey, we encourage you to reach out to your facility contact to ensure that the end date was not added in error.

If any of your in-center hemodialysis (ICH) facility clients are planning to add any new facility-specific supplemental questions to the 2018 ICH CAHPS Fall Survey, please send those questions and their proposed placement to the Coordination Team via e-mail at ichcahps@rti.org on or before 5:00 PM Eastern Time (ET) on August 10, 2018.

Please note that facility-specific questions that were approved by the Coordination Team for use in previous survey periods do not need to be approved again unless the questionnaire wording or response options have changed.

The ICH CAHPS Team is pleased to introduce the ICH CAHPS Coordination Team Semi-Annual Review (CTSAR). This newsletter is a new way to provide in-center hemodialysis (ICH) facilities and survey vendors with ICH CAHPS information.

The CTSAR will be posted on the ICH CAHPS website and updated every January and July. You can navigate to the July 2018 newsletter located under the General Information tab on the ICH CAHPS website or click here to view it now.

The CTSAR will not replace any source of information currently on the ICH CAHPS website; instead, it will highlight important information for readers prior to each survey period. We encourage you to visit the ICH CAHPS website for more information on the items mentioned in this newsletter. Please continue to monitor the ICH CAHPS website for updates and announcements.

Dialysis facilities and other stakeholders have recently expressed an interest to the Centers for Medicare and Medicaid Services (CMS) in a CAHPS Survey for home hemodialysis, peritoneal dialysis, and pediatric dialysis patients. CMS asked RTI International (RTI) to examine the feasibility of conducting a CAHPS Survey of these patient populations.

RTI established a 10-member Techinical Expert Panel (TEP) and conducted a telephone conference with TEP members on March 16, 2018. The TEP was comprised of doctors, nurses, social workers, and patients with expertise in home hemodialysis, peritoneal dialysis, and/or pediatric dialysis. During the TEP telephone conference, TEP members were asked to provide input on a number of issues, including domains of care that would be important to home hemodialysis, peritoneal dialysis, and pediatric dialysis patients. RTI project staff also sought input on domains of care and issues related to a possible CAHPS Survey of kidney transplant patients.

This is a friendly reminder that the deadline for ICH CAHPS Survey vendors to submit the annual Quality Assurance Plan (QAP) is 11:59 PM Eastern Time on March 31, 2018.

The QAP must describe how the survey vendor will implement, comply with, and provide oversight of all survey and data processing activities associated with the ICH CAHPS Survey. To ensure prompt review and acceptance of your Annual QAP, please follow the procedures listed below:

Locate the version of your most recently approved QAP; that is, the version that was accepted by CMS and the Coordination Team in 2017. Make a copy of that version and accept all tracked changes, creating a clean copy of your QAP. Make all 2018 updates to your clean copy using track changes, so that your document reflects only the changes made since your most recently approved QAP. Make sure to include updates to your QAP if your organization has had changes in ICH CAHPS Survey personnel, changes to protocols or procedures, and/or changes...

ICH CAHPS Survey results based on combined data from the 2016 ICH CAHPS Spring and Fall Surveys were posted on Dialysis Facility Compare on the www.medicare.gov website in January 2018 (postposed from October 2017). Click here to see the state and national averages for the three ICH CAHPS composite measures and the three global ratings based on the combined data from those two survey periods. The average response rate for each state and the overall response rate are also provided in this document.

As you know, there has been a delay in providing the 2017 ICH CAHPS Fall Survey sample files to vendors due to a problem with the CROWNWeb files. CMS and the Coordination Team now have the CROWNWeb files and are working diligently to draw the samples for the 2017 Fall Survey.

Below is the revised tentative schedule for the 2017 Fall Survey. Note that we anticipate having the sample files available for vendors to download by 5:00 PM Eastern Time on October 9, 2017. If any changes are required due to issues with the sampling process, we will alert vendors as soon as possible.

In May and June 2017 the ICH CAHPS Coordination Team conducted site visits with some of the ICH CAHPS Survey vendors. Overall, the site visits went well, and both CMS and the Coordination Team appreciate the time and effort vendors put into preparing for the site visit.

A list of questions asked during the site visits and answers provided is included below for your reference. Please review the questions and answers and contact the ICH CAHPS Coordination Team via e-mail at ichcahps@rti.org or call (866) 245-8083 if you have any questions or need further clarification.

The ICH CAHPS Coordination Team is providing state and national averages for three ICH CAHPS global ratings and three composite measures from the 2015 ICH CAHPS Surveys. These state and national averages are now available here.    ICH CAHPS Survey vendors and ICH facilities should be advised of the following when using these state and national averages.



The state and national averages are a weighted average of the 2015 Spring and 2015 Fall ICH CAHPS Surveys. Scores are weighted using the number of respondents for each survey period, thus for a given state the survey period with more respondents will contribute more to that state’s average score. The average scores for the ratings and composite measures are patient-mix adjusted. The state averages are based on survey responses from all sample patients included in both the 2015 Spring Survey and 2015 Fall Survey who received hemodialysis from an ICH facility in that...

As announced in an e-mail message sent to ICH CAHPS Survey vendors on August 1, 2016, the ICH CAHPS Coordination Team has changed the look and feel of the ICH CAHPS website. Most of the changes made were cosmetic; however, there were a few changes to key website operations and functions. A summary of the major changes to the ICH CAHPS website is listed below.

The most recent announcements about the ICH CAHPS Survey will now appear on the home page instead of being available via the “New/Announcement” link in the “Quick Links” box. The ‘Quick Links’ and “For Facilities’ boxes have been consolidated, and will appear together on the right side of the screen. All of the sub-menu items (links to the survey materials and the ICH CAHPS Survey Administration and Specifications Manual, Version 4.0) under the “Survey and Protocols” tab have been collapsed into one menu. All of these items are still accessible under the same tab; however, the sub-menu items will no longer be displayed....

The purpose of this e-mail message is to update all ICH CAHPS Survey vendors about the Kidney Disease Quality of Life (KDQoL) Survey that the Centers for Medicare & Medicaid Services (CMS) is sponsoring for the Comprehensive End-Stage Renal Disease (ESRD) Care (CEC) Model. Earlier this year a CMS contractor conducted the first round of this survey with a sample of ESRD patients who receive dialysis from facilities participating in the CEC Model. CMS’ contractor for the evaluation of the CEC Model will begin KDQoL data collection activities for ESRD patients in non-participating facilities in late June 2016.

Whereas the ICH CAHPS Survey collects data about the patients’ experience receiving hemodialysis care, the KDQoL Survey is designed to collect data from ESRD patients about how dialysis care makes them feel and how dialysis impacts their lives. Both surveys are important for helping CMS understand and improve dialysis care. Because a census of patients is selected each survey period on the ICH CAHPS...

The name of each in-center dialysis facility (CCN) included on the sample file provided by the ICH CAHPS Coordination Team is the name specified on the CROWNWeb file as the facility’s “DBA” (During Business As) name. We provide the DBA name instead of the legal name as these can differ significantly; some sample patients might not recognize a facility’s legal name. In most cases, sample patients should recognize the DBA name that is provided to you on the sample file. However, we strongly encourage you to check with your facility clients to determine the facility name that patients will know or recognize. If that facility name is different from the DBA name, you should include it in the cover letters, mail questionnaires, and telephone scripts.

The ICH CAHPS Coordination Team would like to clarify that the deadline for ICH CAHPS Survey vendors to submit the annual Quality Assurance Plan (QAP) is 5:00 PM Eastern Time on March 31, 2016. Survey vendors must submit their updated QAP by accessing the Submit Quality Assurance Plan option under the “For Vendors” menu on the grey bar at the top of the website’s home page.

The QAP must be updated and submitted annually on or before March 31 of each year and at any time the vendor has a change in ICH CAHPS personnel, capabilities, or systems that are used to administer the ICH CAHPS Survey. The QAP must describe how the survey vendor will implement, comply with, and provide oversight of all survey and data processing activities associated with the ICH CAHPS Survey.

The purpose of this e-mail message is to alert all ICH CAHPS Survey vendors that the Centers for Medicare & Medicaid Services (CMS) is sponsoring a survey called the Kidney Disease Quality of Life (KDQoL) Survey for the Comprehensive End-Stage Renal Disease (ESRD) Care (CEC) Initiative. The sample for this survey, which will be conducted twice each year, will consist of approximately 13,000 ESRD patients of facilities participating in the initiative. CMS’s contractor for that survey will begin data collection activities for the first semiannual survey in February 2016, and the second survey collection period will begin in April 2016.

Whereas the ICH CAHPS Survey collects data about the patients’ experience receiving hemodialysis care, the KDQoL Survey is designed to collect data from ESRD patients about how dialysis care makes them feel and how dialysis impacts their lives. These state and national averages are now available here. They are provided to survey vendors to use for comparative purposes when they provide results to their in-center hemodialysis (ICH) facility clients.

ICH CAHPS Survey vendors should be advised of the following when using these state and national averages.

The state and national averages are based on data from only one survey period (the 2014 ICH CAHPS Survey). The average scores for the ratings and composite measures are patient-mix adjusted and weighted. The ICH CAHPS Coordination Team would like thank ICH CAHPS Survey vendors for their suggestions for changes to the ICH CAHPS Survey materials and clarifications for some of the procedures that vendors made during the data quality telephone conference calls this spring and summer.  Some of the recommendations for changes to the survey materials or in survey protocols and procedures are provided below, along with the Coordination Team’s decisions related to those suggestions.

The sampling window for the 2015 ICH CAHPS Fall Survey includes patients who received hemodialysis from ICH facilities in April, May, and June 2015. The Coordination Team recently learned that patient information for the month of June will not be available from CROWNWeb until early September 2015. Therefore, provision of the sample files to survey vendors and the beginning of the data collection activities for the 2015 Fall Survey will be delayed. The revised data collection schedule and the dates for some key activities related to the 2015 Fall Survey are shown below.

If CROWNWeb is delayed any further in providing patient-level data needed for sample selection, we may need to revise the schedule again. We will let you know in early September whether the samples can be provided as shown in this schedule.

We have discovered a programming issue that impacted a small number of patient surveys at certain facilities participating in the 2015 In-Center Hemodialysis Consumer Assessment of Healthcare Providers and Systems (ICH CAHPS) Spring Survey.

As a result, additional patient surveys will be administered for these facilities to help ensure we have the best information possible to support the goals and objectives of CMS’ program to assess the experiences of in-center hemodialysis patients.

If you are an ICH facility and you were not contacted about this work, no action is required. If your facility was contacted about the additional surveys and you have questions, please contact the ICH CAHPS Coordination Team at ichcahps@rti.org or 1-866-245-8083.

It has been brought to our attention that some in-center hemodialysis (ICH) facilities and ICH CAHPS Survey vendors are receiving calls from some sample patients reporting that they received a survey asking about an ICH facility from which they have never received outpatient hemodialysis. As you know, the sample was drawn from data provided by CROWNWeb. Some vendors experienced this issue during the 2014 ICH CAHPS Fall Survey; at which time the Coordination Team worked with CROWNWeb to determine the reason this issue occurs. At that time, we learned that this issue could be caused by a number of reasons, including changes in facility ownership or status (e.g, from public to private) or an issue with data entry (incorrect CCN entered in the CROWNWeb database), or for some other reason. We will continue to work with CROWNWeb to determine what, if anything, we can do to avoid this issue in upcoming survey periods.

ICH CAHPS Survey vendors must not give the ICH CAHPS Coordination Team’s toll-free telephone number to any patients who call your organization. The results from the national implementation of the In-Center Hemodialysis CAHPS (ICH CAHPS) Survey for each participating in-center hemodialysis (ICH) facility will be publicly reported on a rolling basis on the Dialysis Facility Compare (DFC) website at http://Medicare.gov and will be based on data from two survey periods. The first survey period of the ICH CAHPS national implementation was conducted in the fall of 2014. Data collection for the second survey period (the 2015 ICH CAHPS Spring Survey) will begin in late March 2015 and data will be submitted to the ICH CAHPS Data Center late July 2015. Once public reporting begins, survey results that are publicly reported will be based on data from the two most recent survey periods.

The Centers for Medicare & Medicaid Services (CMS) has not yet decided when public reporting of ICH CAHPS Survey results on the DFC will begin. However, CMS will provide each participating facility with its own survey results for review before the results are publicly reported. Vendors and ICH facilities should check the ICH CAHPS website on a regular basis for more information about public reporting of ICH CAHPS Survey results.

ICH CAHPS Survey vendors must notify the Coordination Team if they learn that one of their in-center hemodialysis (ICH) facilities has closed or will be closing. If the ICH facility closes or will be closing after the Coordination Team has provided a sample for that facility but before the data collection period begins, the survey vendor authorized to collect data on behalf of that facility must complete and submit the online Facility Closing Attestation Form as soon as possible after learning that the facility has or will be closing.

The survey vendor must not begin data collection efforts for that facility. Instead, the vendor should delete all of the personally identifying information provided on the sample file for the closed facility, as per Item 6 of the vendor’s data use agreement (DUA) with CMS. If the facility closed before data collection began (therefore no data were collected) an XML file for this facility should not be submitted to the Data Center.

Please note that ICH CAHPS Survey vendors can only administer the ICH CAHPS Survey using the data collection modes for which they have been approved by the ICH CAHPS Coordination Team. If a vendor would like to offer a data collection mode for which it has not been approved, the vendor must update its Vendor Application and resubmit for review by the Coordination Team.

In addition, if any vendor plans to use a subcontractor that was not included on the vendor’s original vendor application, the vendor must update its vendor application to include the required information about the subcontractor. Please note that any subcontractors that were not included on the vendor’s original application must register for and attend the Introduction to the ICH CAHPS Webinar Training Session that will be held on February 11, 2015.

Some sample patients included in the 2014 ICH CAHPS Survey sample have reported to either the ICH CAHPS Survey vendor and/or to their ICH facility that the ICH CAHPS Survey questionnaire and cover letter that they received contained the name of an ICH facility from which they are not currently receiving or have never received hemodialysis treatments.

If a sample patient calls a survey vendor to report that the questionnaire asks about an ICH facility from which he or she is no longer receiving or has never received hemodialysis treatments, the vendor’s ICH CAHPS Survey staff should instruct the sample patient to answer Questions 1 and 2 in the questionnaire. In Question 2, the sample patient should mark Response Option 5 (I do not currently receive dialysis at this dialysis center) and then skip to Question 45 and answer the remaining questions in the questionnaire.

ICH CAHPS Survey vendors and their ICH facility clients should note that the sample facility for each patient is the facility where that patient received hemodialysis during the sampling window, which for the 2014 Survey is from April 1, 2014 through June 30, 2014. In some situations, the sample patient may have moved and is no longer receiving care at that facility. In other instances, the name of a facility may have changed and the sample patient does not recognize the name of the facility printed in the questionnaire. There may be some other reasons that the facility name that appeared on the CROWNWeb file is not the name of the facility from which the patient received hemodialysis during the sampling window. The Coordination Team is currently investigating this issue.

The online Discrepancy Notification Report (DNR) is now available on the ICH CAHPS Survey website. ICH CAHPS Survey vendors must complete and submit a DNR within 24 hours of discovering an unplanned deviation from the ICH CAHPs Survey protocols. Survey vendors can access the DNR via the Vendor Dashboard on the ICH CAHPS website. Instructions for completing the form are provided on the DNR.

Important announcement for ICH CAHPS Survey vendors and their in-center hemodialysis (ICH) facility clients.

The In-center Hemodialysis (ICH) CAHPS Coordination team is alerting survey vendors and their ICH clients of a potential delay in providing the sample files for the CY2014 In-Center Hemodialysis CAHPS (ICH CAHPS) Survey.

As per the ICH CAHPS Survey Administration and Specifications Manual, Version 2.0, and the ICH CAHPS Vendor Update training session (July 16, 2014), the ICH CAHPS Coordination Team is scheduled to upload the facility sample files by August 25, 2014. ICH CAHPS Survey vendors, in turn, will download the sample file and begin data collection by mailing the prenotification letter to sample patients on September 15, 2014.

The potential delay in providing the sample files is the result of a problem in the CROWNWeb data base. CROWNWeb is the CMS administrative database providing patient-level information from which the ICH Coordination Team will select the patient sample for...

The protocols and guidelines described in Version 2.0 of this manual are effective immediately and supersede those described in the Manual that was posted on the ICH CAHPS Website in January 2014. An electronic version of the Version 2.0 manual in both MS Word and PDF formats is available on ICH CAHPS Survey website. To conserve paper, the ICH CAHPS Survey Coordination Team will print a limited number of hardcopy versions of this manual. As noted in an e-mail message sent to ICH CAHPS Survey vendors on June 3, 2014, the deadline for submitting facility-specific supplemental questions to the ICH CAHPS Coordination Team for review and approval is 5:00 PM Eastern Time on June 30, 2014.

Please note that the Coordination Team will accept facility-specific supplemental questions for review after the June 30th deadline; however, approval of those questions may not be granted in time for the questions to be included in the CY2014 ICH CAHPS Fall Survey. Survey vendors should work with their client ICH facilities to complete the vendor authorization and submission of facility-specific questions as soon as possible to avoid a delay in approval of those questions.

The ICH CAHPS Coordination Team has added some additional information to the Addendum to the ICH CAHPS Survey Administration and Specifications Manual, which was posted on the ICH CAHPS and sent to all ICH CAHPS Survey vendors on May 12, 2014. The revised and new information is included in red font in the Revised Addendum along with today’s date. The revised addendum is posted on the ICH CAHPS Website, and is being sent to all approved ICH CAHPS Survey vendors via email.

The survey vendor application submission and approval process for survey vendors to administer the calendar year (CY) 2014 In-Center Hemodialysis CAHPS (ICH CAHPS) Fall Survey has now closed. The vendor registration form and application are no longer available on the ICH CAHPS website.

Both the vendor registration form and the survey vendor application will be available on the ICH CAHPS website approximately six (6) weeks prior to the next Introduction to the ICH CAHPS Survey Webinar Training Session, which is tentatively scheduled to take place in November 2014. Survey vendors interested in becoming a CMS-approved vendor should check the ICH CAHPS website beginning in September 2014 for an announcement that the application is available.

The Centers for Medicare & Medicaid Services (CMS) is currently conducting a randomized mode experiment as part of the In-Center Hemodialysis CAHPS (ICH CAHPS) Survey with a sample of patients who are receiving in-center hemodialysis to determine whether they respond differently to the survey based on data collection mode. In addition, data from the mode experiment will be used to determine which, if any, patient characteristics affect the ratings of the care they receive. If needed, during the national implementation of the ICH CAHPS Survey, CMS will develop models to statistically adjust survey results before comparative results are publicly reported. Comparative results from the ICH CAHPS mode experiment will not be publicly reported.

The Centers for Medicare & Medicaid Services (CMS) will begin the national implementation of the In-Center Hemodialysis CAHPS (ICH CAHPS) Survey in calendar year (CY) 2014. In-center hemodialysis facilities must contract with a survey vendor that has been approved and trained by CMS to administer the survey on their behalf in CY2014 as part of the requirements for the ESRD quality incentive program (QIP) for payment year (PY) 2016 and beyond.

Survey vendors seeking to become a CMS-approved ICH CAHPS Survey vendor must complete the following initial steps before vendor training occurs:
