When selecting samples for the ICH CAHPS Survey, the Coordination Team uses patient-level information on the CROWNWeb (CW) database and excludes patients that do not meet survey-eligibility criteria, such as patients who reside in nursing homes (also known as skilled nursing facilities), if this is readily known. However, because CW does not contain an explicit indicator that the patient lives in a nursing home, this determination is usually made by vendors during the ICH CAHPS data collection period.

Some vendors have asked for guidance regarding how to handle the situation where the telephone interviewer reaches a nursing home when trying to call a sample patient. Currently, the interviewer has to determine whether a sample patient:

lives in skilled nursing home and therefore receives dialysis at the nursing home (ineligible for the survey), lives in a rehabilitation section or in an assisted living facility (eligible for the survey), or only works at the nursing home but lives elsewhere (eligible for the survey)....

The ICH CAHPS Coordination Team has made some minor changes to the Spanish ICH CAHPS Survey Telephone Script. The changes are listed below for your easy reference. ICH CAHPS Survey vendors should note that these changes must be made in their CATI systems before the 2018 Fall Survey begins (and they should begin using them in the 2018 ICH CAHPS Fall Survey).

Q2: Added a question mark to the end of response option 4 in Question 2. Response option 4 now displays as, “5 años o más?” instead of the previous text, “5 años o más”. Q20: Updated response option 1 in Question 20 to display as “Un injerto o “graft”,” instead of the previous text, “Un injerto,”. The addition of the text “o graft” has been made to better assist telephone interviewers with selecting the correct response option for situations where the survey participants reply with “graft” instead of “injerto” to this question. Q59: Updated response option 4 in Question 59 to display as “Asiática, o” instead of the previous text, “Asiática,”...

The ICH CAHPS Coordination Team has made a change to the ICH CAHPS Survey mail questionnaire. All ICH CAHPS Survey vendors must make this change in their survey materials before the 2018 ICH CAHPS Spring Survey begins in April 2018.

As noted in prior survey periods, the OMB number must appear as a stand-alone number on the front cover of the mail questionnaire (or the first page of the questionnaire if there is no cover), even when the OMB disclosure notice is also printed on the questionnaire cover. Starting with the 2018 Spring Survey cycle, the OMB expiration date is required to be printed under the stand-alone OMB number. For reference, the OMB expiration date is February 29, 2020.

The final ICH CAHPS Survey mail questionnaire in all approved languages is now available on the ICH CAHPS website:  https://ichcahps.org/SurveyandProtocols.aspx.

The Coordination Team has made a minor change to the ICH CAHPS Survey Telephone Script to clarify the appropriate use of the two intro statements that appear above Q45. Please note that telephone interviewers should read only one of these intro statements to a respondent, as follows.

The Q45_INTRO should be used when completing the telephone interview with an eligible respondent. That is, the sample patient reports in Q1 that he or she gets dialysis from a dialysis center and he or she has received dialysis for 3 months or more in Q2. After the Q3-44 have been administered to the respondent, the interviewer should read the Q45_INTRO. Once the Q45_INTRO is read, the CATI system should go to Q45, as indicated in the new skip instruction, which has been added to the script.

Please note that the Coordination Team has made a wording change to Q39 in the Spanish ICH CAHPS Mail Questionnaire and Telephone Script. We are now using a different Spanish term to refer to the word “belly.” ICH CAHPS Survey vendors should note that this change must be made in their mail questionnaire and CATI systems before the 2017 Fall Survey begins.

Please note that the Coordination Team has made some minor changes to the ICH CAHPS Survey Telephone Scripts. The changes and the reasons for the changes are described below. ICH CAHPS Survey vendors should note that these changes must be made in their CATI systems before the 2017 Spring Survey begins (and they should begin using them in the 2017 ICH CAHPS Spring Survey).



Added the phrase “Would you say” to Question 16: During site visits to survey vendors, the Coordination Team noticed that some respondents used one of the response options in the “Never” to “Always” scale when they answered Q16. Because the Never to Always scale is used in most of the questions that precede Q16, the respondent assumed that the same scale would be used in Q16. To address this issue, we have added the phrase “Would you say…” at the end of Q16, and changed the “Yes” and “No” response options to lower case, which means that the interviewer should read the response options for Q16 to the respondent....

When first contacted by telephone, some ICH CAHPS sample patients from CCNs that are phone-only or mixed-mode might begin the telephone interview but for some reason are unable to complete the interview during that call. The ICH CAHPS Coordination Team has added INTRO3 and INTRO4 for telephone interviewers to use when they call sample patients back to complete the interview. Note that Intros 3 and 4 are to be used in a callback to complete the survey with sample patients who answered at least one question in the survey in a previous call and who indicated during that call they are willing to complete the interview later. As you know, patients who began the interview but then refused to continue at a certain point or hung up after the interview began are considered refusals, and should never be called back. These new introductions are for patients who would like to complete the interview but could not during a previous call.

Please note that after reading INTRO3 and INTRO4, the CATI program should skip the telephone interviewer to the first unanswered question in the survey. Updated telephone scripts in both English and Spanish can be found here. The changes below should be implemented for the 2016 Fall Survey.

This email serves to remind ICH CAHPS Survey vendors who use telephone interviewing systems that have auto-dialing capability that they must follow all FCC-required procedures as described on page 54 of the ICH CAHPS Survey Administration and Specifications Manual, V 4.0. We have copied those procedures below for your reference.

Predictive or automatic dialers are permitted, as long as they are compliant with Federal Trade Commission (FTC) and Federal Communications Commission (FCC) regulations, and as long as respondents can easily interact with a live interviewer. FCC regulations prohibit auto-dialing of cell phone numbers. Therefore, cell phone numbers need to be identified in advance to allow the vendor to treat cell phone numbers in a way that complies with FCC regulations. It is the vendors' responsibility to familiarize themselves with all application state and federal laws and abide by those accordingly in regard to calling cell phone numbers. The ICH CAHPS Coordination Team has revised the ICH CAHPS sample mail cover letters for both the first and second questionnaire mailings. The new sample cover letters give specific instructions for sample patients who are receiving home or peritoneal dialysis to skip to Question 45.

The final sample mail cover letters in all approved languages are now available. Survey vendors can use the sample cover letters, or they may choose to develop their own cover letters, provided that the cover letter requirements described in the ICH CAHPS Survey Administration and Specifications Manual are met.

The ICH CAHPS Coordination Team has made one change to the ICH CAHPS telephone survey script in both English and Spanish. INTRO_2 now allows the telephone interviewer to introduce themselves and the vendor when speaking with the actual sample patient. The prior scripts only included this introduction in response to someone asking who was calling. This change should be implemented for the 2016 Spring Survey.

INTRO2     Hello, this is [INTERVIEWER NAME] calling from [VENDOR].  [ICH Facility Name] is taking part in a national survey to learn more about the quality of care patients receive from their hemodialysis center. Your name was selected at random from among people who receive in-center hemodialysis care by the Centers for Medicare & Medicaid Services to participate in this survey. The Medicare program and dialysis centers will use survey results to help improve the quality of hemodialysis care they provide.

Your participation in this survey is voluntary and will not affect your health benefits...

A revised XML file layout is now available here on the ICH CAHPS website. This document reflects the changes made in the questionnaire and telephone script that will be used in the 2015 ICH CAHPS Spring Survey, including the deletion of the gender and age questions and the change to the wording of the question about language spoken in the home.

Please note that this XML file layout replaces the version used during the 2014 Fall Survey and also the 2015 Spring Survey version that was originally uploaded in early February and removed from the website shortly thereafter. Please note that the English-language telephone interview script that was recently posted on the website was missing the “M Missing/DK” option for Q57. We have added that response to the telephone script and posted the revised script on the here.

Note that the “M Missing/DK” option is included in the Spanish-language version of the telephone interview script that was posted on the ICH CAHPS website in early February 2015.

Please note that the Spanish-language telephone interview script that was posted on the website recently was missing the skip instruction for response option 8 in Q57. When revising your CATI program for the Spanish-language telephone script to reflect the changes made to the ICH CAHPS Survey questionnaire that will be used in the 2015 Spring Survey, make sure that the program will skip to Q57a if the sample patient chooses response option 8 in Q57. The revised Spanish-language telephone script is located on the Survey Materials page.

Note that this skip instruction is included in the English-language version of the telephone interview script that was posted on the ICH CAHPS website in early January 2015.

The ICH CAHPS Coordination Team has made changes to the ICH CAHPS mail survey questionnaire, telephone survey script, and the OMB Disclosure Notice. All ICH CAHPS Survey vendors must make these changes in their ICH CAHPS Survey materials before the 2015 ICH CAHPS Spring Survey begins in March 2015. The final mail survey questionnaire, telephone script, and OMB Disclosure Notice in all approved languages are now available here.

A revised XML File Layout that reflects the changes to the revised questionnaire will be available on the website within the next few weeks. Please note that these updated materials, including the revised XML File Layout, are for the 2015 Spring Survey.

The following is a list of changes that have been made to the ICH CAHPS Survey materials that will be used in the CY2015 ICH CAHPS Spring Survey.

Survey vendors should note that the Coordination Team is in the process of revising the ICH CAHPS Survey Questionnaire that will be used during the 2015 Spring Survey. The revised mail survey questionnaire will be sent to all ICH CAHPS survey vendors via email today. The same changes will be made to the telephone script; however, we are in the process of revising the interviewer introduction in the telephone script, so the revised version will not be available until early January.

We are notifying ICH CAHPS Survey vendors of these changes now so that they can begin making changes to their survey materials and associated computer programs.

Changes to the ICH CAHPS Survey.

The age (Q56) and gender (Q57) questions will be deleted. The wording in Q59 (How well do you speak English?) will be revised to read “What language do you mainly speak at home?” The response options for Q59 will be as follows. English Spanish Chinese Samoan Russian Vietnamese Portuguese...

Survey vendors should note the following changes to the ICH CAHPS Survey materials and protocols.

Race Questions in the Telephone Script. We have made some changes to Question 62 (race question) in the ICH CAHPS Survey Telephone Script so that it is comparable to the race question in the mail survey questionnaire. The mail questionnaire contains one race question that allows a sample patient to choose all race categories that apply. In May, we revised the telephone script to include one gate question for race and two follow-up questions (Q62a and Q62b) if the sample patient chose Asian or Native Hawaiian/Pacific Islander in the gate question. However, the follow-up questions in the telephone script currently only allow the sample patient to choose one group that best describes them. Telephone survey respondents must have the same option to choose two or more race categories as do mail survey respondents, therefore we have revised Q62a and Q62b in the telephone script to make those questions comparable to the mail survey.

Please note that the phone script has been updated to include a skip instruction to the DON’T KNOW response option in Q20 and Q37. If the sample patient does not know the answer Q20, the telephone interviewer must enter response option 4 for DON’T KNOW, at which point your CATI program should skip to Q22. Similarly, if the answer to Q37 is DON’T KNOW, the CATI program should skip to Q39 after response option 3 is entered in Q37. The revised telephone scripts in both English and Spanish are now available.

As indicated in an announcement posted on the ICH CAHPS Survey Website on February 25, 2014, the ICH CAHPS Coordination Team is in the process of finalizing the ICH CAHPS Survey mail questionnaire and telephone script. We would like to give potential ICH CAHPS Survey vendors the opportunity to suggest edits or changes to the questionnaire, other survey materials, or the ICH CAHPS Survey specifications and protocols.

If you have any suggested edits or changes to the ICH CAHPS survey questionnaire or specifications that will be used in the national implementation, please send your suggestions to the Coordination Team via email message to ichcahps@rti.org no later than 5:00 PM Eastern Time on March 31, 2014.

Survey vendors seeking to become a CMS-approved ICH CAHPS Survey vendor should note that the ICH CAHPS Survey questionnaire and telephone script posted on the ICH CAHPS website are not yet considered final. The Coordination Team is currently changing Question 60a for both mail and telephone, and Question 61a for telephone. In addition, other minor changes may be made in the near future. The Coordination Team expects that the questionnaire and telephone script that will be used in the CY2014 ICH CAHPS Survey will be final within the next few weeks. An announcement will be posted on the ICH CAHPS Website when the final mail survey questionnaire and telephone script are posted.

The following changes have been made to the In-center Hemodialysis CAHPS (ICH CAHPS) Survey questionnaire that will be used in the CY2014 ICH CAHPS Survey.

The title of the survey on the cover page of the questionnaire has been changed to “Medicare In-Center Hemodialysis Survey.” The Office of Management and Budget Paperwork Reduction Act Disclosure Notice is now on the cover page. Survey instructions that include the sample facility name and how to handle skip patterns have been moved to the cover page. In Question 1, the skip pattern for the “At home” response option has been changed. Sample patients who indicate that they now receive dialysis at home are instructed to skip to Q45. The wording in Q2 has changed; the question now reads “How long have you been getting dialysis at [SAMPLE FACILITY NAME]?” In Q2, the skip instruction for the “Less than 3 months” response option has changed. Sample patients who choose this response option are now instructed to skip to Q45. In Q2, the skip instruction for the “I do not currently receive dialysis at this facility” response option has changed. Sample patients who choose this response option are now instructed to skip to Q45....

Some in-center hemodialysis facilities and survey vendors have asked whether they should use the ICH CAHPS Survey Questionnaire that was posted on the Agency for Healthcare Research and Quality’s (AHRQ’s) ICH CAHPS Website (at https://www.cahps.ahrq.gov/hemodialysis/ ) or the survey that is posted on this Website when conducting the ICH CAHPS Survey in calendar year (CY) 2013.

CMS recommends that the ICH CAHPS Survey Questionnaire posted on the AHRQ ICH CAHPS Website be used for surveys conducted in CY2013. Survey vendors may use the questionnaire version that is posted here. Please note that the About You section in the ICH CAHPS Survey Questionnaire might change before national implementation of the ICH CAHPS Survey begins in CY2014 as part of the requirements for the ESRD quality incentive program (QIP) for payment year (PY) 2016 and beyond.

The ICH CAHPS Survey (questionnaire) in Traditional Chinese and Simplified Chinese have been posted on this website. Revised versions of the ICH CAHPS Survey in English and Spanish have also been posted on this website. The revisions made are as follows.

  • The transition statement that applies to Questions 3-9, which was originally included in Question 3, has been moved to appear above Question 3.
  • The transition statement that applies to Questions 10-25, which was included in Question 10, has been moved to appear above Question 10.
  • The transition statement that applies to Questions 26-31, which was included in Question 26, has been moved to appear above Question 26.
  • Q61 has been revised.
