Change in protocol for including the CMS logo on mailed documents

The current ICH CAHPS Survey protocol for prenotification letters requires that survey vendors print “Centers for Medicare & Medicaid Services” along with the vendor’s return address in the left top corner of the prenotification mailing envelope. In addition, “Return Service Requested” or “Change Service Requested” or “Address Service Requested” must also be included.

Beginning with the 2015 Fall ICH CAHPS Survey, all ICH CAHPS Survey vendors must begin including the CMS logo in the return address section of the prenotification letter envelope to alert sampled patients that the mail packet is being sent to them by CMS. The CMS logo will replace the “Centers for Medicare & Medicaid Services” title. If you have already printed the prenotification envelopes for the 2015 Fall Survey, please submit an Exceptions Request Form asking that your organization be permitted to make this change beginning with the 2016 Spring Survey instead.

In addition, all ICH CAHPS Survey vendors that are administering the survey using mail or mixed modes now have the option of placing the CMS logo on the survey mailing envelopes, if desired.

The ICH CAHPS Coordination Team will send each Survey Administrator a copy of the CMS logo in a separate email. This logo must be used only for the purposes specified above.
