Deadline for Completing Facility Non-Participation Form is February 15, 2015

As noted in previous announcements posted on the ICH CAHPS website, ICH facilities that served 29 or fewer survey-eligible patients in calendar year (CY) 2014 are not required to participate in the ICH CAHPS Survey in CY 2015. However, all such facilities may administer the survey in 2015 if they choose to do so.

ICH facilities that authorized a survey vendor on the ICH CAHPS website in CY2014 must notify the Coordination Team that they WILL NOT be administering the ICH CAHPS Survey in CY2015 by completing and submitting the Facility Non-Participation Form on the ICH CAHPS website. The deadline for submitting the Facility Non-Participation Form is 11:59 PM Eastern Time on February 15, 2015.

Within the next few weeks, the ICH CAHPS Coordination Team will be selecting and distributing a sample to the ICH CAHPS Survey vendor that each facility authorized on the ICH CAHPS Website. If your facility is not planning to administer the ICH CAHPS Survey in 2015, please make sure that you complete the online Facility Non-Participation Form by the deadline so that a sample file is not distributed to the survey vendor that you authorized previously on the ICH CAHPS website.

To complete the Facility Non-Participation Form, ICH facilities must first log into the ICH CAHPS website. Once logged in, place the cursor over the “For Facilities” tab on the navigation bar at the top of the home page, then click the “Facility Non-Participation Form” link.

If your facility is planning to administer the ICH CAHPS Survey in 2015 and has already authorized an ICH CAHPS Survey vendor on the ICH CAHPS website, no action is needed on your part.
