Updated ICH CAHPS Mail Questionnaire, Telephone Script, and OMB Disclosure Notice Now Available

The ICH CAHPS Coordination Team has made changes to the ICH CAHPS mail survey questionnaire, telephone survey script, and the OMB Disclosure Notice. All ICH CAHPS Survey vendors must make these changes in their ICH CAHPS Survey materials before the 2015 ICH CAHPS Spring Survey begins in March 2015. The final mail survey questionnaire, telephone script, and OMB Disclosure Notice in all approved languages are now available here.

A revised XML File Layout that reflects the changes to the revised questionnaire will be available on the website within the next few weeks. Please note that these updated materials, including the revised XML File Layout, are for the 2015 Spring Survey.

The following is a list of changes that have been made to the ICH CAHPS Survey materials that will be used in the CY2015 ICH CAHPS Spring Survey.

Changes to the ICH CAHPS Mail Survey Questionnaire

1. “What is your age?” has been deleted (Q56).
2. “What is your sex?” has been deleted (Q57).
3. Q59 “How well do you speak English?” has been revised to read “What language do you mainly speak at home?”
   English
   Spanish
   Chinese
   Samoan
   Russian
   Vietnamese
   Portuguese
   Some other language (please identify)
4. “Do you speak a language other than English at home?” and “What is that language?” have been deleted (Q60 and Q60a).
5. The OMB Disclosure Notice has been updated to include a specific mailstop at CMS.
6. Due to these changes, question numbers have changed.

Changes to the ICH CAHPS Telephone Script

1. The telephone script introduction has been shortened.
2. “What is your age?” has been deleted (Q56).
3. “What is your sex?” has been deleted (Q57).
4. Q59 “How well do you speak English?” has been revised to read “What language do you mainly speak at home?”

1. The OMB Disclosure Notice has been updated to include a specific mailstop at CMS.
2. The Simplified Chinese OMB Disclosure Notice has been updated to reflect correct language.
