Determining Whether Participation in the ICH CAHPS Survey in CY2015 is Required

To determine whether your ICH facility (CMS Certification Number or CCN) is required to participate in the ICH CAHPS Survey in calendar year (CY) 2015, count the number of survey-eligible patients your facility served in CY2014. Your count must include hemodialysis patients who:

• were 18 years old or older as of December 31, 2014;
• were alive as of December 31, 2014;
• received hemodialysis on an outpatient basis from your facility for 3 consecutive months or longer at some point in CY2014;
• are not currently receiving hospice care; and
• do not currently live in a nursing home or other skilled nursing facility or other long-term facility such as a prison.

ICH facilities that served 29 or fewer survey-eligible patients in CY2014 are not required to participate in the ICH CAHPS semiannual surveys in CY2015. However, all such facilities may administer the survey in 2015 if they choose to do so.

ICH facilities that will not be administering the ICH CAHPS Survey in CY2015 must complete and submit the online “Facility Non-Participation Form”, which will be available on the ICH CAHPS website beginning January 15, 2015.

ICH facilities that received Medicare certification after December 31, 2014 are not required to participate in the ICH CAHPS Survey in CY2015. Newly certified facilities are not required to complete and submit the online Facility Non-Participation Form.
