The CY2015 In-center Hemodialysis CAHPS (ICH CAHPS) Survey

Any in-center hemodialysis (ICH) facility that served 30 or more survey-eligible patients in calendar year (CY) 2014 must administer the ICH CAHPS Survey in the spring and fall of CY2015 to comply with quality reporting requirements for the 2017 payment year (PY) .

ICH facilities should note that the ICH CAHPS Coordination Team DOES NOT have information about the number of survey-eligible patients a facility served in CY2014. Therefore, each ICH facility is responsible for determining whether it is required to administer the 2015 ICH CAHPS semiannual surveys.

ICH facilities that choose not to administer the ICH CAHPS Survey in CY2015 based on having fewer than 30 survey-eligible patients in CY2014 must notify the Coordination Team by completing an online Facility Non-Participation Form, which will be available on the ICH CAHPS website from January 15 through February 15, 2015. More information about the online Facility Non-Participation Form will be sent to all ICH facilities on or before January 15, 2015 via an e-mail message.

Unless a Facility Non-Participation Form is submitted, the Coordination Team will select and provide a sample file to the ICH CAHPS Survey vendor that the facility previously authorized on the ICH CAHPS website. If an ICH facility plans to use a different ICH CAHPS Survey vendor, the facility must update its vendor authorization form on the ICH CAHPS website on or before 5:00 PM Eastern Time on February 15, 2015.
