***REPLACEMENT Sample Files for the CY2014 ICH CAHPS Survey Ready for Download***

Replacement sample files for the 2014 ICH CAHPS Survey are now available on the ICH CAHPS website for download by survey vendors who have been authorized by one or more in-center hemodialysis (ICH) facilities to submit ICH CAHPS Survey data on their behalf.

ICH CAHPS Survey vendors will have until 11:59 PM Eastern Time (ET) on Wednesday, September 17, 2014 to attest that they have taken ownership of and have downloaded their replacement sample file.

To download and open the replacement sample file:

• Login to the ICH CAHPS website using your user credentials.
• Under the “Data Submission” tab, click the Sample File Download option.
• Read the attestation statement and agree to it by clicking the box that appears to the left of “I agree.”
• Click on the link to the sample file to download it.
• Once the file is downloaded and saved on your computer system, use the password that was sent by email to your ICH CAHPS Survey Administrator to open and decrypt the file.

The Coordination Team will send a separate email containing a Replacement Sample Summary Report to each survey vendor. After you receive the Replacement Sample Summary Report, please review the sample file that you downloaded in conjunction with the Replacement Sample Summary Report.

If you are missing sample information for a CCN, or note any other issues with your sample file, please contact the ICH CAHPS Coordination Team via e-mail at ichcahps@rti.org or call 1-866-245-8083 as soon as possible.

Please note that the data collection schedule will remain the same with prenotification letters being mailed on September 29, 2014. If you feel that you cannot meet the current data collection schedule, please let the Coordination Team know as soon as possible.
