Sample Files for the CY2014 ICH CAHPS Survey Ready for Download

Sample files for the 2014 ICH CAHPS Survey are now available on the ICH CAHPS website for download by survey vendors who have been authorized by one or more in-center hemodialysis (ICH) facilities to submit ICH CAHPS Survey data on their behalf.

ICH CAHPS Survey vendors will have until 11:59 PM Eastern Time (ET) on Tuesday, September 9, 2014 to attest that they have taken ownership of and have downloaded their sample file.

To download and open the sample file:

• Login to the ICH CAHPS website using your user credentials.
• Under the “Data Submission” tab, click the Sample File Download option.
• Read the attestation statement and agree to it by clicking the box that appears to the left of “I agree.”
• Click on the link to the sample file to download it.
• Once the file is downloaded and saved on your computer system, use the password that was sent by email to your ICH CAHPS Survey Administrator to open and decrypt the file.

Please carefully review your sample file to verify that you have a sample for each CCN that has authorized your organization to submit data on its behalf. When reviewing the sample file, please note a sample could not be selected when:

• the CROWNWeb file did not have any patient-level data for a CCN; and
• none of the patients for whom information was included on the CROWNWeb file met survey eligibility criteria.

The Coordination Team will send an email containing a Sample Summary Report to each survey vendor. This report shows the number of patients sampled for each CCN that authorized the vendor to submit data on its behalf, including CCNs for which no patients were sampled.
